The Help That I Need... And Do I Really Need It ... (And What Is It Anyhow?)...?


AT THE WEEKEND, after I'd got a bit over-exuberant and what she likes to call "really manic" but I would categorize as "a bit hypomanic" (no way was I "really manic" in doctorly terms; I was just a bit elevated in mood; a little hyper) my friend Binky told me I really need more help from the mental health services than I'm currently getting. It's true the first thing she did on my arrival was to insist on laundering my clothes, which I thought were clean but she definitely didn't. Then I had a shower in her heated bathroom, which meant that I spent longer than a couple of seconds in there. And on coming out and changing into grey jogging bottoms and a fuschia pink teeshirt that she insisted was red but it wasn't ~ it was most definitely pink ~ I have to admit, I felt a whole lot cleaner. That is, cleaner as in less dirty. Not altogether clean. I never am and never feel "clean". But maybe that's my inner drug-addict being honest, hey.

One of the Support Workers who works at the Supported Housing where Binks lives listened as I told the story of how I ever came to the attention of the mental health system at all. That was to do with a drought in the UK's heroin supply and a sudden attack of mixed mania and psychosis coinciding with the "first day of the rest of my life"... that is, my first day clean of the terrible weak and adulterated heroin that was going around in those days. And how I never really recovered over the following weeks. Because I was keeping a diary with a four-point mood-scale going upwards and down and was scoring +1.5 and -1.5 on the same day. +1.5 means a very noticably elevated mood and hyperkinesis similar to the effects of speed. -1.5 is a very sour, depressive mood. +2 on this scale means full-blown mania, +3 is psychotic mania, +4 is a quasi-delirious state with disorientation and confusion ~ as bad as mania gets. The very maximum on each end would be a plus or minus five.

In the first flush of psychotic mood disorder I went up to about +3.5 and down below -4. I remember the aerial falling off the top of my TV and it staying that way, with barely any picture, for two days because the television was merely an object upon which to fix as I stared into space. After about six weeks of rolling moods I suddenly went sky high and this is where I hit a +4.99, about as high as you can go.

But these days I'm still scoring plus and minus 1.5s. I was +1.5 on Saturday afternoon. My self and my house are getting ever further into disarray. [I never stay high; it's the change of direction that throws me every time. I never know where I am, where I'm going.] Binky somehow knows that my living space is in dire need of clearing and cleaning, even though she's never set eye on it. She needs a knee replacement and so rarely walks further than the nearest busstop, and when we meet at home it's always her place.

In a moment of empassioned despair I went and telephoned Naomi, the lady who used to run the Dual Diagnosis "Nutter Club" (as I called it). I'm not her patient and she knows I'm only phoning for advice. I would never expect practical help from her: she's far too snowed under by all her other cases... She returned my call this morning saying the best thing I can do, to get more help, is say to my GP that I need a psychiatric referral ~ or more to the point, to enquire as to where the current one has got to. When I turned up feeling depressed about two months ago, she said she was referring me to a psychiatrist. The other option is to ask for help via the methadone clinic; but Naomi underlined for me how prejudiced psychiatrists can be against drug users (that must mean they're prejudiced against most of their patients as most people with ongoing mental health issues these days are drug-takers, if not full-blown drug-addicts like me...)

She reminded me of stuff I suppose I already knew in my heart: that if you want help for depression, for example, you shouldn't downplay the "suicidal ideation" nearly all depressives get. I just don't like talking about stuff like that; unless I really feel bad in the moment I mention it, I nearly always feel separated from my own feelings and myself when I do so, so there's a good chance I'd have to hold myself back from laughing. I can't take myself seriously the way I'm "supposed to". I just can't. Well I'll try... but I wonder what I think this doctor can do..? I don't want any more meds or drugs. The one thing I think might help is counselling... I mean, I hate to admit this, as counselling has always been the knee-jerk response of health professionals of all persuasions... but who knows; maybe it would help...? It's true I still feel traumatized by the mere fact that I went totally fruitloops barking bonkers in early 2011. I certainly do not feel I've had any closure on this issue.

Binky says that what I need is the same manner of Supported Housing she lives in. And that I need a social worker and a thing called a CPA which means a Care Plan Approach ~ a written contract-type billing of what treatment I can expect and call upon when and from whom, especially in emergency. Because as far as I'm aware I have nothing like this. No community nurse I can get in touch with. All I know about handling emergency situations is that I'm meant to present myself to the nearest mental hospital's emergency department. Where they seem deliberately to keep everyone waiting for hours, as if the long waiting time is going to put anybody off when actually all it does is severely annoy some already annoyed people and help further to unravel others who have already passed their wits' ends.

So really, No. No true help is available anywhere. Maybe I would do better in a Dual Diagnosis Service (geared towards mentally deranged drug addicts) rather than the one I go to, which seems to be geared to the needs of people whose main issue is the drugs. Giving up the drugs never seemed to help my mental states in the past, which is why I'm somewhat doubtful that just giving up heroin is really going to do much good to my mind. It'll probably do my body far more favours. The two street drugs most associated with mental ill health and addiction are cocaine and speed in all their forms, neither of which I've touched in I don't remember how long. In many cases, cannabis is probably worse for a person's mental health than anything else, including crack. Which is a big reason why I loathe the stuff with such passion.


All that spliff-toking has ever done to me over the past few times, scattered as they were over many years, was to bring on paranoid psychotic symptoms without any redeeming features (such as elevated mood). The last time I smoked cannabis was a complete accident that happened because I'd been collecting cigarette butts from a nearby bus-stop and found what I'd taken for a nice fat rollie and not really noticed the herbal flavour until my mind was already enrobed by amnesiac paranoia. Then there's alcohol and "alcohol is a depressant so that's what's probably making you depressed" as many people told me... So how come I've felt equally bad, and sometimes more so without the drink..?  I don't think heroin helps me... Naomi did say this morning that she thought I'd probably been self-medicating bipolar disorder for years with heroin; and yes it did used to seem to stabilize my moods. Which it doesn't do any more. So apart from its inefficacy, I want rid of that stuff because as a member of my family once pointed out, it has killed my creativity. And it certainly has. In the early days of addiction, heroin might make a person feel more creative. But I don't think they usually are any more creative, in terms of the number of things created and their quality...

Naomi asked what my exact label was and when I told her "schizoaffective" she said she feels I'm far more towards the bipolar side of that diagnosis than the schizo one. Which puts her in accord with everyone else I know. Neither I nor any of my friends think of me as "schizophrenic". In fact the only person ever to use that expression has been my GP, who doesn't seem to perceive any meaningful difference between schizophrenic and schizoaffective. Well from what I've seen in others there's a wide gulf separating the two. Schizophrenia is an ongoing psychosis characterized by disorders of thought. Schizoaffective bipolar disorder, which I'm supposed to have, is an extreme disorder of moods with some schizophrenic features. There's a second type of schizoaffective disorder, which Binky's supposed to suffer from, which involves severe depression on top of schizophrenia. The doctors seem to have successfully medicated most of her depression away but schizophrenic features persist. If you get her talking on the right subjects, she can sound completely delusional...

Binky also says some weird things that are basically her shit. Eg that if I read too much about my own alleged condition the doctors will alledge that what I'm telling them is the result of my researches, not my experience. Well this cannot be true as I wasn't well enough to pick up any knowledge about what schizoaffective disorder actually was until I'd recovered enough from last year's episode to be clear-headed enough to actually take any information in. And between that time and this I haven't seen any psychiatrists at all. Also some of my most extreme experiences are barely touched upon by any modern texts ~ I only saw them described in Victorian textbooks I was able to access online. Plus the way I'd describe my experience and the way it tends to be expressed by others are very different. Example: I have experienced my thoughts exploding into starbursts. I've never heard anyone describe it that way. Other people talk of "racing thoughts", but that symptom never happens to me until the mania is so severe I'd be having difficulty communicating. Binky talks of racing thoughts she says she experiences in the night but I cannot relate to whatever it is she means. A person who comes home in the early hours of the morning only to find they've lost their keys might characterize their thoughts as "racing" ~ but that's nothing at all like the racing thoughts of mania, which are literally in such extreme fast-forward you can barely catch hold of a single one without it exploding into scores of others skedaddling in all directions with the utmost rapidity. So it's almost impossible to say what you're thinking about at all ~ the subject has changed so many times, the original point totally lost. You can't even remember where you've been, let alone how you got there...

Ho-hummm you see I have got on this subject YET AGAIN. And WHY? Because there is no closure. All I want is some validation and maybe some explanation... of what on earth it is that has been happening to me.

As for this "help" that I supposedly need... what help? When? How? And WHY?? ;-)  :-(  :-)

American-style Crisscross Fries


I COULDN'T FIND "American-style curly fries" last time I looked in Iceland ~ no I'm not a jetset food tourist: Iceland is the about the cheapest food shop in Britain. Especially if like me you have really chavvy taste. So instead of curly I went crisscross ~ and they are deliciously vulgar. I can really imagine eating them on the 18th floor of a towerblock in the flat next to a crackhouse. The type of block where the lifts stink of skunk and the only gold in sight (that isn't round the drugdealer's neck) is on packets of Benson and Hedges cigarettes. It's not all bad living in the sky. You do get really amazing TV reception up there.

(I don't smoke Benson and Hedges: they're too expensive for me. I smoke Berkley or Sterling Superkings, broken into 3 and rolled into 3 separate mini-cigarettes, complete with Swan mini-filters. This way I can go an entire 24-hours on just "10" cigarettes (made into 30).

When I'm really broke, I pick up dog-ends from the bus stop, but you have to be careful when you find what looks like a really juicy discarded roll-up. Several times I've imbibed cannabis this way. Cannabis is the drug I loathe beyond all others. It's like paranoid schizophrenia and Alzheimer's rolled into a skin. Last time I smoked it (and that really was by accident) I was hearing voices within about 4 tokes. It's nasty nasty stuff. Psychosis without the manic, elevated mood.

Anyway what was I talking about. O yeah crisscross fries. I eat mine with battered cod chunks and chopped tomatoes doused in black pepper and vinegar.

Does anyone follow Real Housewives of New York City? Yesterday I finally caught up with enough to be able to follow the story. Isn't that LuAnn a bitch when she's angry? Very beautiful though. How old is she? Does she have great genes or a great plastic surgeon, or both? She was having a massive row with Alex over some inter-housewife hiccup on their marakesh trip. She's the kind of girl you have to go several rounds with in the verbal fighting before you get to shoot every point she makes down in flames.

I just found out she was born in 1965, so she's about 47..?? Anyway I feel sorry for people appearing in those docusoaps. What it's easy for the viewer to forget is that whenever you end up in a fight with another character, you're not just interested in winning the fight. Your entire social standing and reputation could depend on the outcome. So no stress then.

American TV producers have a weird grasp of the word "housewife". Here it means a stay-at-home wife and/or mother who spends her days doing minor chores between tea and biscuits and turgid TV of the kind I watch...

Anyway I can't wait till I get foreign telly piped in. I'm so fed up of the English crap it's unreal.

I haven't touched heroin in days. 15 or 16mg methadone, whatever the dose is, holds me perfectly. I make sure I drink it in the early afternoon ~ which means there's little to no risk of waking in the morning feeling sick and not wanting to face the walk to the chemists plus an hour-+ wait after drinking to feel OK again.

Still deaf... and a post on drugs, addiction, heroin, crack and mephedrone (meow meow), psychosis, depression, bipolar and other aspects of post-addiction mental health ...

THE "MENTAL" SECTION starts after the ***asterisks***...


BECAUSE of my infected ear and continuing deafness, I have made a second doctor's appointment on Friday. Thursday was out becauseI've a hospital appointment with the Consultant who will hopefully explain to me why I am growing a left breast and turning into a woman. I found out last week that my regular GP is still in training. She told me my eardrum is most definitely not ruptured, but seemed surprised that I was not in pain due to the extent of the infection. My hearing in that ear is very muffled. I have tinnitus loud enough to hear when I'm out and about during the day ~ above the traffic and even above the hubbub of screaming little brats and foreign students who come into the library for the free internet. I remember a time when the only type of communication permitted in such places was stage-whispering and even then one was liable to being sternly shushed. I have to say, I find whispers ten times more annoying than murmured conversation. Neither of which I would probably be able to follow in my current state of decrepitude. I dropped the wallet containing all my plastic cards today and didn't notice. Thankfullly I found them all scattered across the floor in my doorway. Had I dropped them out of doors I wouldn't have had a clue. I definitely need this ear treated. The warning noticed on the products I have been using are doing my head in. Otex (peroxide drops) for breaking down my ear wax are not to be used in cases of ear infection, which stressed me because although I felt no pain, I did suspect an infection, but had no way of confirming whether or not this was the case. Gut feeling said I was infected, and the doctor of course confirmed this. The present antibiotic spray insists it is not to be used in cases of perforated eardrum, which the doctor says I haven't got, but instinct tells me I have. So I've goto to wait three days to find out whether I've been inadvertently harming myself all the more. A quick Google on the issue told me that use of the wrong spray can actually cause nerve damage! The doctor's first choice wasn't even available and won't be stocked anywhere for at least two weeks. The dark side is telling me I won't ever get my old hearing back, so I might as well learn to live with dropping crucial possessions, mishearing and not hearing, and the sensation that half my head is stuffed up with damp and putrefying worm-wriggling cotton wool.

 Even if the infection does miraculously clear by Friday, I'm afraid the deafness won't. I'm wondering when it ever will.

 A drug-dealer called about an hour ago. When I said I didn't want his wares I'm sure he called me the c-word before hanging up. But even in my good ear, my hearing is so bad, I'll never know ...

Instead of wasting another £20 with him, I went round Morrisons for stuffed pasta and cheese, cod in parsley sauce with finely chopped broccoli, chicken saag (that's Hindi for spinach) masala and naan bread, Heinz baked beans and baby Hovis wholemeal bread and cloudy lemonade (contains acesulfame K and aspartame on top of sugar, but barely any soft drinks I know don't...) and so on and so on. Two full-to-the-brim carrier bags for £21.50.

Then into the library and a perusal of Christopher Ciccone's Life with My Sister Madonna, a memoir of unmatched bitterness. Starting off as her backing dancer, then "dresser", a job he says entailed little more than picking her sweat-drenched underwear off the floor, he went on to design interiors for her multiple homes and to help plan her world tours. The book is a catalogue of Madonna's inconsiderate behaviour. The biggest irony is that her marriage to the British film director, Guy Richie, that put the biggest wedge in their relationship, was all but over by the time this book was published. he took the money ~ and surely it was a six-figure sum ~ and any chance of a reconciliation with his sister was permanently scuppered.

This gradual methadone detox of mine ~ five days ago I dropped from 25mg a day to 20 ~ is going too fast. I have constant indistinct cravings for heroin and, contrary to my resolution, have used it after the Queen's Diamond Jubilee, twice. So I'll think back to this year as "the time I was giving up heroin", rather than the time I gave it up. Ho-hum...

I know I cannot use on top of such a tiny script . Any use of heroin now is helping me mess things up royally. I can't get my head round the fact that I can never take that crap ever again. True, I'm falling out of love with the "B", as we call it here, more and more as the weeks sail by ~ but the relationship is never over. Long ago I got my head around the fact that "your last hit is the one you never take" ~ meaning that for the rest of my life, heroin shall probably remain in the category of Unfinished Business ...

Then again, I have an enormous collection of drugs relegated to the trashcan of times past: Ecstasy, magic mushrooms, ketamine, crack ... every mindbending substance in fact, bar heroin. Actually, if the druggiepixie could magic them into my hand, none of these substances, with the possible exception of crack, because I only gave up regular use three years ago, and because most of the dealers sell crack as well as heroin and because most hard drug addicts in this country are actually more into crack than heroin, if the truth be told... none of the old drugs are any temptation any more. The last time I took anything remotely like them was when a heavily personality-disordered acquaintance [he had borderline personality disorder] sorted me out a free sample of some candy-odoured white powder he declared to be a "legal high". If it's legal it can't be any good, I opined. Oh, this is great stuff, he assured me. But he was very vague when I tried to find out what it was called. it's like E and coke without any comedown, was as much as he would say. Thee-and-a-half minutes after snorting a weighed fifth of a gram in two nose-burning lines, I was already high with a euphoria that climbed and climbed for another twenty minutes, plateauing out for three hours, then gradually diminishing to leave my feet back on the ground. it was like Ecstasy, except with a real kick to it that MDMA lacks. Incidentally, it's this quick kick that tends to make drugs more addictive. I felt weird for days afterwards. Then I found out it had been 4-methylmethcathinone ~ mephedrone, which is a chemical cousin of the intoxicant in the Somali chewing leaves known as qat (or khat) ~ hence the media-invented street name "meow". Mephedrone was banned by the British government in April 2010 ~ too late for the man who had supplied me. Trapped in a cycle of escalating compulsive self-administration, he'd developed a florid paranoid psychosis. Allowed out of the psychiatric ward for "a walk", he'd ended up at the King's Cross tube station where, following a suicidal impulse, he flung himself in front of an underground train and died. Crippled by loneliness and a dearth of mephedrone following the ban that April, his best friend (my friend "Perky") who had a history of speed psychosis and post-opiate withdrawal depressive psychosis, locked herself away and took all the psychiatric medication and all the methadone she could get her hands on, all at once. And she was found dead in her room.

That's two people I know who were killed by a "harmless legal high". Just knowing that has been the final nail in the coffin of my Chemical Mystic Years. It's no coincidence that I think that if anything kindled the full-on psychotic episodes I had last year, it was the psychedelic and Ecstasy-type drugs with which I experimented in my 20s that caused the damage ~ not the heroin and crack to which I was addicted in my 30s. I believe the psychological damage caused by heroin takes the form of an acquired weakness. Just as a healthy person who confines themself to a wheelchair for ten years will gradually waste away, eventually becoming unable to walk; so a person who relies on heroin to survive life will become psychically vulnerable, so that predisposed individuals put themselves at risk of psychotic episodes in the months after heroin and/or methadone detox. I went psychotic after the switch from heroin to methadone (proof, if any more were required, that methadone is no true substitute for heroin). My late friend Perky, spiralled down into psychotic depression after giving up the methadone ~ several years before she finally killed herself, having relapsed and  ended up on methadone yet again. I've haerd of a lot of people who developed psychotic illness after coming off opiates. This seems to be because bipolar disorder and schizophrenia tend to first become manifest in the late teens and early adulthood ~ precisely the same time a person is most likely to incline towards drug-experimentation. Opiates do seem to have some antipsychotic effect [see links below], meaning that in vulnerable people, the mental condition can remain masked for as long as opiate addiction continues. When the opiates are withdrawn, the severe mental illness ~ which was always there in the background ~ comes blazing to the fore, giving the impression that heroin or methadone caused this illness, when in actuality they held it back for many years.

Many of the life stories I've heard in NA talk about severe mood swings in the first few months lived "clean". But this doesn't mean every newly detoxed addict is "bipolar". Classic full-blown type 1 bipolar disorders involves swings from clinical depression, where appetite and sleep are markedly affected ~ not just mood ~ to clinical mania, which usually has psychotic features along with the other hallmark symptoms such as severely racing thoughts, extreme distractibility, physical agitation and/or hyperactivity (which means it's practically impossible to do a 90-minute NA meeting without constantly going outside to pace back and forth and smoke cigarettes) accompanied by great garrulousness, grandiosity and/or paranoia and a marked decrease in the need for sleep, so that many manic people can go for days on end with no sleep at all. A truly manic person is very obviously "high" and/or extremely agitated and irritable. If you turn up to an NA meeting in such a state, people will naturally assume you're on something. They certainly did with me much to my vexation!

There is a less extreme form of bipolar, known as type 2, where the patient never develops full-blown mania but instead has episodes of milder hypomania, as well as clinical depression. This is the fashionable type of bipolar disorder that celebrities like Catherine Zeta Jones go down with. It never involves psychotic episodes and, in America at least, is often diagnosed retrospectively, without the doctor actually witnessing the patient in a hypomanic state. It is this type of bipolar disorder that many critics suspect is being overdiagnosed.

Bipolar ii is still far more severe than the mood swings experienced by the average recovering addict. Many addicts, in my experience, do suffer from "emotional problems" which are frequently both the cause and effect of their addiction. When these problems are extreme enough to warrant psychiatric attention, they tend to take the form of anxiety and depression, not psychosis. In fact, anxiety and depression, with a touch of obsessive-compulsive behaviour, is what I suffered from in the early years. I've always been prone to paranoia, too. Of course cannabis-smoking never helped there. I only smoke ordinary cigarettes today since I got addicted to the tobacco component in spliffs ...

Is it true, by the way, that Americans roll their joints with marijuana only? Wouldn't this blow your head off? I remember finding a big bag of weed in a nightclub. It happened to be lying next to the Aphex Twin, Richard James, who was DJing that night. I told my friend Tommy Tired "I've got the Aphex Twin's grass!" Of course I don't know it was his, or even whether the Aphex Twin has ever been into grass. Suffice it so say that Tommy, a heavy smoker, had run out of fags. So we went for a walk from Hoxton through the City of London (the financial district), puffing on incredibly potent skunk-only spliffs. I was so stoned I heard my most private thoughts reflected and echoed off the great glass panes of the empty office buildings. We got so "mislaid" it took hours to get home. that was the last bag of grass I ever owned, and that was back in 1996 or 1997. I'd already given up smoking cannabis on anything like a regular basis back in 1993.
